CMS Finalizes Favorable TAVR Coverage Rules, Boosting Outlook for Edwards Lifesciences

The Centers for Medicare & Medicaid Services (CMS) on Friday published its finalized national coverage determination (NCD) for Transcatheter Aortic Valve Replacement (TAVR), formalizing changes that are expected to benefit the procedure's adoption. Analysts at Stifel described the final rules, which closely followed a June proposal, as "constructive" for both the overall TAVR market and market leader Edwards Lifesciences (EW).
Key Policy Updates
The finalized decision affirms several key updates that streamline procedural requirements and expand patient eligibility. According to Stifel, these changes are a net positive for the TAVR market.
- Expanded Coverage: The new rules confirm Medicare coverage for TAVR in patients with asymptomatic severe aortic stenosis, a key factor that prompted the NCD review.
- Procedural Efficiency: CMS will now require only one TAVR operator to be present in the operating room, a change from previous two-person requirements. Stifel notes this provides a significant "scheduling efficiency benefit."
- Team-Based Approach: While procedural requirements were eased, CMS maintained the need for a multi-disciplinary "heart team," including input from both interventional cardiology and cardiac surgery, to evaluate patient suitability. Procedures must also be performed at facilities with cardiac surgery backup.
- Evidence Requirements: The "coverage with evidence development" requirement has been eliminated for most severe aortic stenosis patients, though it will still apply to the newly covered asymptomatic patient group.
Market and Company Impact
AdThe updated CMS coverage is significant for a TAVR market that Stifel values at over $7 billion globally. The procedure is a critical revenue driver for Edwards Lifesciences, accounting for approximately 75% of the company's revenue, according to the firm's analysis. In contrast, it represents about 5.5% of Medtronic's (MDT) revenue.
Stifel stated that the finalized NCD supports Edwards' long-range plan for revenue growth exceeding 10%. The positive outlook is also bolstered by the company's product pipeline and favorable competitive dynamics.
Future Coverage Analysis
In a concurrent announcement, CMS revealed it is initiating a new national coverage analysis for TAVR devices designed to treat aortic regurgitation. This review, with an expected completion date of June 8, 2027, could potentially open a new market for devices like Edwards' clinical-stage J-Valve and the FDA-approved JenaValve from a privately held competitor.
