The Oncology Institute Named Top Pick by B. Riley After Debt Refinancing

Background

B. Riley Securities has named The Oncology Institute (TOI) its "Best Idea" from its recent May conference, reiterating a Buy rating on the healthcare services company. The endorsement follows TOI's successful refinancing of a major debt obligation, a move the investment firm believes removes a key financial overhang and improves its financial positioning.

A Key Overhang Removed

The central reason for B. Riley's optimistic outlook is TOI's recent handling of its $86 million in convertible debt, which was originally due in August 2027. The company has now settled this balance without raising additional equity, a move that prevents dilution for existing shareholders.

According to the firm's report, the refinancing was accomplished through:

  • A new $75 million term loan provided by OrbiMed.
  • An $11 million payment from TOI's existing cash balance.

The new term loan extends the company's financial runway, with a maturity date in 2031 and an anticipated interest rate between 9% and 12%.

B. Riley maintained its $8 price target on TOI's stock. Analysts Yuan Zhi and Liwen Wang stated that with the financing uncertainty resolved, investors can now refocus on what they describe as the company's high-growth healthcare services business, potentially leading to a "re-rate" of the stock.

The analysts also pointed to the company's recently improved free cash flow, which showed a $20 million improvement at the midpoint of its 2026 guidance. For the upcoming second quarter, B. Riley projects revenue of $152.8 million, slightly below the consensus estimate of $155.2 million, as it models stronger growth in the latter half of 2026.

Company Guidance and Market Context

TOI's management has reiterated its long-term growth ambitions, targeting 20% annual revenue growth over the next few years. The company aims to reach $1 billion in revenue by 2028, with $50 million to $60 million in adjusted EBITDA.

Other Wall Street firms have also issued positive updates. Lake Street recently initiated coverage on TOI with a Buy rating and a $10 price target, while BTIG raised its price target to $9 and Needham increased its target to $7.
