Ionis Pharmaceuticals Stock Falls After Cardiovascular Drug Fails Phase 3 Trial

Shares of Ionis Pharmaceuticals (NASDAQ: IONS) tumbled 13.1% in pre-open trading to $50.51 following the failure of its high-profile cardiovascular drug, pelacarsen, in a late-stage clinical trial. The negative result eliminates a major potential revenue stream and compounds recent pipeline setbacks for the biotechnology company.
Pivotal Trial Misses Endpoint
The sell-off was a continued reaction to a September 4 disclosure from Ionis's partner, Novartis, that the Phase 3 Lp(a)HORIZON study for pelacarsen did not meet its primary endpoint. The six-year trial, which enrolled more than 8,300 high-risk patients, failed to demonstrate a statistically meaningful reduction in the composite risk of cardiovascular death, heart attack, stroke, or urgent coronary revascularization compared to a placebo.
Critically for investors and the scientific community, while pelacarsen successfully lowered levels of the biomarker lipoprotein(a), this biological effect did not translate into improved health outcomes. The result raises fundamental questions about the therapeutic hypothesis of targeting Lp(a) to treat cardiovascular disease.
Financial Fallout and Market Context
The trial's failure has significant financial implications for Ionis. A 2019 licensing agreement with Novartis is now void of its major commercial potential, which included:
Ad- Up to $650 million in development, regulatory, and commercial milestone payments.
- Tiered royalties on net sales in the mid-teens to low-20% range.
This news follows another sharp sell-off in July 2026 after a separate Phase 3 cardiovascular trial for the drug eplontersen also failed. With Tuesday's pre-market decline, Ionis shares are approaching their 52-week low of $50.01, far from the 52-week high of $86.74.
Analyst Outlook
Despite the setback, some analysts view the impact as manageable. According to Investing.com, analysts at Citi acknowledged the failure but argued that Ionis's broader pipeline limits the immediate downside. Another analyst reportedly maintained a Buy rating, characterizing the miss as a "contained blow."
Investors will now focus on the company's remaining assets, including the recently approved ZANVASTRO and the upcoming PDUFA catalyst for zilganersen on September 22, for future growth drivers.
