Intuitive Surgical Tumbles as Slowing U.S. Growth Overshadows Q2 Earnings Beat

Summary
Shares of Intuitive Surgical fell sharply after the company reported a slowdown in its core U.S. da Vinci procedure growth, sparking investor concern despite second-quarter earnings and revenue that surpassed analyst estimates.
Intuitive Surgical (NASDAQ: ISRG) stock plunged in pre-market trading Friday, falling 9.5% to $364.27 after its second-quarter earnings report highlighted a significant deceleration in a key growth metric. The sell-off occurred even as the robotic surgery leader beat Wall Street expectations, signaling that investors are focused on the sustainability of its future growth.
A Beat Isn't Enough
For the second quarter of 2026, Intuitive Surgical posted adjusted earnings of $2.80 per share on revenue of $2.89 billion. These results topped consensus analyst forecasts of $2.51 per share and $2.82 billion in revenue, according to Investing.com.
However, the market's attention was fixed on the slowdown in U.S. da Vinci procedure growth, which moderated to 12% in the second quarter, down from 14% in the first quarter. On an earnings call, management attributed the softening to the expiration of enhanced Affordable Care Act (ACA) premium subsidies and a pullback in deferrable procedures, which affected when patients sought care.
Cautious Guidance and Market Headwinds
AdAdding to investor concerns, Intuitive Surgical maintained its full-year 2026 procedure growth forecast at a range of 13.5% to 15.5% but guided toward the midpoint. This was interpreted by the market as a conservative signal, suggesting limited upside for the remainder of the year. In response, Stifel cut its price target on the stock to $550 from $670, though it maintained a Buy rating, citing the slowing U.S. procedure environment.
A broader market sell-off amplified the pressure on Intuitive's shares. The Nasdaq fell 2.0% and the S&P 500 declined 1.0% amid a risk-off sentiment affecting high-valuation growth stocks. At a price-to-earnings ratio near 49x, well above its medtech peers, Intuitive's stock is particularly vulnerable to any perceived weakness in its growth story.
Competitive Landscape in Focus
The results also renewed focus on the competitive landscape. Investors are increasingly weighing the long-term threat from rivals as Medtronic's Hugo system is now commercially available in the U.S. and Johnson & Johnson's Ottava robot advances toward potential FDA authorization. These developments challenge the near-monopoly that Intuitive Surgical has long enjoyed in the robotic-assisted surgery market.
