They are vying for a piece of a sector that analysts project could reach $100 billion in annual sales within the next decade, fueling a wave of innovation and strategic deal-making.

Market Leaders Fortify Their Pipelines

Incumbents Eli Lilly and Novo Nordisk are not standing still, advancing next-generation treatments to protect their market share. Lilly has seen success with its oral pill orforglipron (Foundayo) and is developing retatrutide, which in one key trial helped patients lose more than 28% of their body weight over 18 months.

Novo Nordisk is developing several experimental drugs, including a next-generation injection called amycretin and CagriSema. While CagriSema produced a 22.7% weight reduction in one late-stage trial, the result was below the company's internal expectations. Both companies are also pursuing licensing deals to bolster their early-stage pipelines.

Big Pharma Enters the Fray

Major pharmaceutical firms are aggressively entering the market through acquisitions and late-stage development. Pfizer is targeting a 2028 approval for its first weight-loss drug, a once-monthly injection that showed up to 12.3% weight loss at 28 weeks in one study. Amgen is conducting six late-stage trials for its drug MariTide, which helped patients shed up to 20% of their body weight in a year-long mid-stage trial.

Other industry giants are using strategic partnerships to gain a foothold. Merck signed a deal worth up to $2 billion for an experimental oral drug from China-based Hansoh Pharma. Similarly, Roche acquired Carmot Therapeutics for $2.7 billion to access its obesity drug candidate, enicepatide, while AstraZeneca is licensing experimental drugs from CSPC Pharmaceutical Group in a deal that could be worth over $17 billion.

Emerging Biotechs and Novel Approaches

Smaller biotech firms are also making significant progress, often with innovative drug candidates that could differentiate them from the competition.

  • Viking Therapeutics reported that its experimental drug VK2735 helped patients maintain up to 97% of their weight loss when dosing was reduced from weekly to every other week.
  • Structure Therapeutics is developing a once-daily pill, aleniglipron, which helped patients lose up to 16.3% of their weight in a mid-stage trial.
  • Scholar Rock is exploring a novel approach by combining its drug, apitegromab, with Lilly's Zepbound to help patients preserve significantly more lean muscle mass during weight loss, addressing a key concern with existing therapies.
