Intuitive Surgical shares plummeted after the company warned that changes to U.S. health insurance plans are softening demand for its da Vinci robotic surgeries, renewing investor concerns about the health of the medical technology sector. The robotic surgery giant's stock fell more than 12% in pre-market trading Friday despite maintaining its full-year procedure growth forecast.

Growth Forecast and Insurance Headwinds

Intuitive Surgical announced it still expects worldwide da Vinci-assisted procedure growth for 2026 to be near the midpoint of its 13.5% to 15.5% forecast. However, the company noted that U.S. procedure growth in the second quarter moderated to 12%, which it said was slower than anticipated at the start of the year.

Management attributed the slowdown, particularly in surgeries that can be deferred, to shifting insurance dynamics for patients. "In our customer conversations, some have said changes in patient coverage and premium dynamics may be affecting when patients seek care and move forward with treatment," said CEO David Rosa on a call with analysts, as reported by Reuters.

The company specifically cited the expiration of enhanced Affordable Care Act (ACA) premium subsidies as having a "modest adverse impact" on its second-quarter U.S. procedure volume.

The cautious outlook from Intuitive Surgical disappointed Wall Street and directly contradicted more optimistic recent commentary from medical device maker Abbott. Abbott had previously described the idea that ACA enrollment declines would materially harm the industry as a "flawed assumption."

Intuitive's warning aligns more closely with an earlier report from HCA Healthcare, the largest for-profit hospital operator in the U.S. HCA had flagged softer demand for surgical procedures and a growing number of uninsured patients following the end of pandemic-era subsidies.

"We thought medtech was set for a relief post ABT (Abbott) print," said Evercore ISI analyst Vijay Kumar, according to Reuters. He added that the softness in Intuitive’s U.S. procedures, the slowest in three years, "rekindles the debate" about underlying demand in the sector.

What It Means for Investors

For investors, the key uncertainty is the precise magnitude of the impact from ACA subsidy expirations on procedure volumes, which are the primary driver of Intuitive's revenue. J.P. Morgan analyst Robbie Marcus noted that while management cannot yet quantify the effect, his firm's sense is that it is a "small percentage impact," according to a research note.
