Intuitive Surgical Stock Falls to 52-Week Low on Softer Procedure Guidance

Shares of Intuitive Surgical (NASDAQ: ISRG) have fallen to a new 52-week low, trading down more than 31% over the past year following a significant post-earnings selloff. The decline came after the surgical robotics leader issued a weaker-than-expected forecast for procedure growth, creating a sharp contrast between the stock's bearish technical indicators and the company's strong underlying financial health.
Guidance Triggers Selloff
The market's negative reaction was driven by the company's full-year 2026 guidance, not its second-quarter performance. In its report on July 17, Intuitive Surgical beat analyst expectations, posting earnings per share of $2.80 versus an estimated $2.51 and revenue of $70 million above forecasts.
However, the company projected full-year da Vinci procedure growth of 13.5% to 15.5%, its slowest pace since 2021. Management attributed the softer outlook to the expiry of Affordable Care Act (ACA) subsidies, which is putting pressure on elective benign surgeries. The company described the impact as "modest and not durable" and did not include a potential recapture of these procedures in its guidance.
Fundamentals Remain Robust
Despite the guidance, Intuitive Surgical's financial position remains exceptionally strong. The company's business model is supported by a growing installed base of 11,710 da Vinci systems (up 12% year-over-year), which drives recurring revenue from instruments and accessories.
AdKey financial metrics underscore the company's stability:
- Balance Sheet: The company operates with zero debt.
- Profitability: It maintains best-in-class margins, with a gross margin of 66.8% and a net margin of 28.4%.
- Capital Efficiency: The company's return on invested capital (ROIC) stands at a healthy 16.8%.
- Liquidity: A current ratio of 6.4x indicates a very strong ability to meet short-term obligations.
Technical Picture and Analyst View
The stock's technical chart currently signals caution for investors, with key indicators showing a strong downtrend. Shares are trading below their previous 52-week low of $378.50, and technical analysis points to potential support levels at $337.45 and a longer-term floor near $321.78.
Wall Street analysts, however, have largely maintained their bullish outlook, viewing the current headwinds as temporary. BMO Capital reiterated its Outperform rating, calling the ACA-related weakness "transient," while TD Cowen maintained a Buy rating. The average analyst price target implies a potential upside of over 40% from current levels, according to data from Investing.com.
